Generic Rally Registration Forms

We have provided 2 generic rally registration forms for download. The purpose of offering both forms is to enable the user to fill out the form on the computer or to fill out the form by hand. Both forms ask for the same information, but it is our intent to suit the needs of every COAA member and non-member wanting to participate in a rally. The word document may be filled out electronically using Microsoft Word (version 97 or newer). This file may be saved and emailed to the rally host as an attachment or may be printed out and mailed to the rally host through the US postal service. The PDF file may be downloaded, printed out and filled out by hand. This can be mailed to the rally host through the US postal service. These generic forms enable any person (member or non-member) desiring to participate in a rally to send his/her information to register for a rally well in advance. If the host requires more specific information, he/she will request this information as needed or will place his/her customized registration form for download within the rally information box above. Please email Jonathan Bopp at jbopp@berry.edu with any additional questions.

Please note that newer versions of Word will open this file in a "protected view" mode. If this is the case, it will be necessary to click the yellow button labeled "enable editing" at the top of the screen. This will allow you to fill in the form.
